Description
Overview of the medicine
Micatel 40 mg tablet contains Telmisartan, an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to reduce the risk of cardiovascular events in certain high-risk patients.
Uses & Indications
Dosage
Adults
Initial dose 40 mg once daily. Max dose 80 mg once daily.
Elderly
No specific dose adjustment is necessary, but a lower starting dose may be considered.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Oral administration. May be taken with or without food, once daily.
Mechanism of Action
Telmisartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation and decreased blood pressure.
Pharmacokinetics
Onset
Antihypertensive activity within 3 hours. Full effect in 4-8 weeks.
Excretion
Primarily excreted unchanged via feces (over 97%). Minor renal excretion.
Half life
Approximately 24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with absolute bioavailability of about 42-58%. Food intake has a minor effect on absorption.
Metabolism
Minimally metabolized to an inactive acylglucuronide. Not metabolized by cytochrome P450 system.

Contraindications
- Hypersensitivity to Telmisartan or any component of the formulation
- Pregnancy (especially second and third trimesters)
- Lactation
- Biliary obstructive disorders
- Severe hepatic impairment
- Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
Digoxin
Increased plasma digoxin concentrations.
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Contraindicated in diabetes or renal impairment; increased risk of hyperkalemia, hypotension, and renal impairment.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium
May lead to hyperkalemia.

Store below 30°C in a dry place, away from light. Keep out of reach of children.

Symptoms may include hypotension, dizziness, tachycardia, or bradycardia. Treatment is symptomatic and supportive; hemodialysis is unlikely to be effective. Closely monitor vital signs.
