Micosone-2-1-cream is a topical antifungal and corticosteroid combination cream used to treat various skin conditions characterized by fungal infections accompanied by inflammation, itching, and redness. It combines the broad-spectrum antifungal action of Miconazole Nitrate with the anti-inflammatory and antipruritic effects of a potent corticosteroid (Betamethasone Valerate).
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Do not use for more than 2-4 weeks without medical advice.
Elderly
Same as adult dosage. Use with caution due to thinner skin.
Renal_impairment
No specific dosage adjustment required for topical use due to minimal systemic absorption.
How to Take
For external use only. Wash and dry the affected area before application. Apply a thin film of cream to cover the affected skin area and gently rub in. Wash hands thoroughly after application unless hands are the treated area. Avoid contact with eyes, mouth, and other mucous membranes.
Mechanism of Action
Miconazole Nitrate inhibits the biosynthesis of ergosterol, a vital component of fungal cell membranes, leading to increased cell membrane permeability and leakage of cellular contents, ultimately resulting in fungal cell death. Betamethasone Valerate, a potent corticosteroid, acts by inducing phospholipase A2 inhibitory proteins, thereby inhibiting the release of arachidonic acid, and subsequently inhibiting the biosynthesis of prostaglandins and leukotrienes. This reduces inflammation, itching, and vasoconstriction.

Drug Interactions
Warfarin
Potentiation of anticoagulant effect, leading to increased bleeding risk, due to Miconazole's effect on CYP2C9. Monitor INR.

Increased risk of systemic side effects if used concurrently on large areas.
Storage
Store below 30°C in a dry place, away from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause acute systemic effects. However, prolonged or excessive use of high-potency corticosteroids can lead to systemic effects like Cushing's syndrome, hyperglycemia, or hypothalamic-pituitary-adrenal (HPA) axis suppression. Treatment is symptomatic and supportive; gradual withdrawal of the corticosteroid may be necessary.
