Description
Overview of the medicine
Progesterone is a female hormone important for the regulation of ovulation and menstruation. Microgest 400 mg vaginal pessary is used in fertility treatments, for maintaining pregnancy, and preventing preterm birth. The vaginal route allows direct delivery to the uterus.
Uses & Indications
Dosage
Adults
Typically 400 mg once or twice daily, vaginally. Dosing starts from the day of embryo transfer or as advised by the physician. Duration depends on the indication and physician's guidance.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with hepatic or renal impairment.
Renal_impairment
Use with caution; no specific dose adjustment guidelines, but monitoring is advised if severe impairment.
How to Take
Insert the pessary deep into the vagina, preferably at bedtime. Wash hands thoroughly before and after use.
Mechanism of Action
Progesterone acts by binding to progesterone receptors in the uterus, vagina, and other tissues. It converts the proliferative endometrium into a secretory one, which is essential for successful implantation and maintenance of pregnancy. It also inhibits uterine contractions.
Pharmacokinetics
Onset
Systemic effects can be observed within hours, local uterine effects are more direct and rapid.
Excretion
Mainly renal, as glucuronide and sulfate conjugates.
Half life
Approximately 16-18 hours (for progesterone generally, depends on route and formulation).
Absorption
Rapidly absorbed from the vaginal mucosa, leading to high local concentrations in the uterus and systemic circulation.
Metabolism
Primarily hepatic, converted to pregnanediol and other metabolites.
