Nortriptyline is a tricyclic antidepressant used to treat depression, various forms of neuropathic pain, and for migraine prophylaxis.
Uses & Indications
Dosage
Adults
For neuropathic pain or migraine prophylaxis, initial dose 5-10 mg at bedtime, gradually increasing as tolerated, usually not exceeding 25-50 mg daily. For depression, typically 25 mg three to four times daily, or a single dose at bedtime, up to 150 mg daily.
Elderly
Lower initial dose (e.g., 5-10 mg at bedtime) and slower titration, maximum dose generally lower (e.g., 75 mg daily) due to increased sensitivity to side effects.
Renal_impairment
No specific dosage adjustment recommended, but caution advised due to potential accumulation of metabolites in severe impairment.
How to Take
Take orally, with or without food, usually once daily at bedtime due to sedative effects. Swallow whole with water; do not crush or chew.

Side Effects
Contraindications

Drug Interactions
MAOIs

Anticholinergics

Storage

Overdose
Symptoms include severe drowsiness, confusion, hallucinations, fast/irregular heartbeat, hypotension, seizures, coma, respiratory depression, and cardiac arrest. Treatment is supportive, including gastric lavage, activated charcoal, and continuous monitoring of vital signs, ECG, and fluid balance.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit outweighs the potential risk to the fetus. Nortriptyline is excreted in breast milk; therefore, use with caution in nursing mothers, considering the benefits of breastfeeding and the potential risks to the infant.
