Description
Overview of the medicine
Flunarizine is a selective calcium entry blocker and histamine H1 antagonist, primarily used for the prophylaxis of migraine and symptomatic treatment of vestibular vertigo and dizziness.
Uses & Indications
Dosage
Adults
Migraine prophylaxis: Initially 10 mg once daily at bedtime for patients under 65 years. For patients 65 years or older, 5 mg once daily at bedtime. After 2-3 months, if symptoms are controlled, reduce to 5 days a week with 2 drug-free days. Vertigo: 5-10 mg once daily at bedtime.
Elderly
A lower initial dose (e.g., 5 mg daily) is recommended due to potential increased sensitivity and risk of side effects.
Renal_impairment
No specific dosage adjustment is generally required as excretion is primarily biliary, but caution is advised in severe cases.
How to Take
Take orally, preferably in the evening/bedtime to minimize daytime drowsiness.
Mechanism of Action
It selectively blocks calcium influx into vascular smooth muscle cells and neuronal cells, particularly under conditions of excessive depolarization. It also has H1 antihistaminic and mild sedative properties.
Pharmacokinetics
Onset
Migraine prophylaxis: Therapeutic effects observed after several weeks of continuous treatment. Vertigo: Faster relief.
Excretion
Mainly excreted in feces via biliary route; minor renal excretion.
Half life
Very long elimination half-life, ranging from 18 to 23 days.
Absorption
Well absorbed orally. Peak plasma levels typically reached in 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2D6 and CYP3A4.
Side Effects
Contraindications

- •History of Parkinson's disease or other extrapyramidal disorders.
- •Known hypersensitivity to flunarizine or any excipients.

CNS depressants (alcohol, sedatives, hypnotics)
May enhance the sedative effects of flunarizine.
Anticonvulsants (e.g., Carbamazepine, Phenytoin)

Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include sedation, agitation, tachycardia. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if overdose is recent.
