Overview of the medicine
Milam (Midazolam) is a short-acting benzodiazepine used for sedation, anxiolysis, and amnesia during medical procedures, as well as for the short-term treatment of insomnia and acute seizures.
Uses & Indications
Dosage
Adults
For insomnia: Initial 7.5-15 mg orally, 15-30 minutes before sleep.
Elderly
For insomnia: Initial 3.75-7.5 mg orally, due to increased sensitivity and reduced metabolism.
Renal_impairment
Dosage reduction may be necessary; use with caution.
How to Take
Take orally with water, preferably before sleep. Do not crush or chew tablets.
Mechanism of Action
Midazolam enhances the effect of gamma-aminobutyric acid (GABA) at the GABA-A receptor, leading to central nervous system depression.

Onset
Oral: 15-30 minutes; Intravenous: 1-5 minutes
Excretion
Mainly renal, as conjugated metabolites.
Half life
1.5-2.5 hours
Absorption
Rapidly and completely absorbed orally, but undergoes significant first-pass metabolism. Oral bioavailability is about 40-50%.
Metabolism
Primarily hepatic, via CYP3A4, to active metabolite α-hydroxymidazolam.
Side Effects
Contraindications
- •Hypersensitivity to midazolam or other benzodiazepines
- •Acute narrow-angle glaucoma
- •Severe respiratory insufficiency
- •Sleep apnea syndrome
Drug Interactions
Opioids
Increased risk of respiratory depression and sedation.
Alcohol and other CNS depressants
Additive sedative effects.
CYP3A4 inhibitors (e.g., ketoconazole, grapefruit juice)
Increased midazolam levels and effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, confusion, impaired coordination, diminished reflexes, coma. Treatment is supportive, and flumazenil may be used as an antidote.
Pregnancy & Lactation
Pregnancy Category D. Use during pregnancy only if the potential benefit outweighs the risk. Excreted in breast milk, use with caution in nursing mothers.
