Description
Overview of the medicine
Minomal-R 175 mg tablet is an extended-release formulation of Minocycline, a tetracycline antibiotic. It is primarily used for the treatment of inflammatory lesions of non-nodular moderate to severe acne vulgaris and for the treatment of rosacea.
Uses & Indications
Dosage
Adults
175 mg orally once daily. Typically for 6 to 12 weeks for acne or as directed by a physician.
Elderly
No specific dosage adjustment is generally required, but caution is advised in patients with renal or hepatic impairment.
Renal_impairment
Use with caution in severe renal impairment; dose reduction may be necessary.
How to Take
Take the tablet whole with a full glass of water. Do not crush, break, or chew. It can be taken with or without food, but consistency is recommended.
Mechanism of Action
Minocycline exerts its antibacterial action by inhibiting bacterial protein synthesis. It binds reversibly to the 30S ribosomal subunit of susceptible bacteria, thereby blocking the binding of aminoacyl tRNA to the acceptor site on the mRNA-ribosome complex. This prevents the addition of new amino acids to the growing peptide chain and inhibits protein synthesis. Its anti-inflammatory effects contribute to its efficacy in acne and rosacea.
Pharmacokinetics
Onset
Antibacterial effects within hours; clinical improvement in acne may take weeks.
Excretion
Excreted primarily through glomerular filtration in the urine and also via feces.
Half life
Approximately 11-26 hours (extended-release formulations).
Absorption
Well absorbed from the gastrointestinal tract, though absorption may be affected by food and dairy products.
Metabolism
Partially metabolized in the liver, forming several inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to minocycline or other tetracyclines.
- Severe hepatic impairment.
- Pregnancy and lactation (especially during the second and third trimesters of pregnancy due to potential for permanent tooth discoloration and bone growth inhibition in the fetus).
- Children under 12 years of age.
Drug Interactions
Warfarin
Tetracyclines may potentiate the anticoagulant effect of warfarin. Monitor INR closely.
Penicillins
Tetracyclines may interfere with the bactericidal action of penicillins. Avoid co-administration.
Isotretinoin
Concurrent use may increase the risk of pseudotumor cerebri. Avoid co-administration.
Oral contraceptives
May decrease the effectiveness of oral contraceptives, leading to breakthrough bleeding or unintended pregnancy.
Antacids containing aluminum, calcium, or magnesium
May decrease the absorption of minocycline. Separate administration by at least 2 hours.
Iron preparations and multivitamins containing iron
May decrease the absorption of minocycline. Separate administration by at least 2 hours.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, discontinue the drug and institute symptomatic and supportive therapy. Gastric lavage may be considered within the first few hours. Minocycline is not significantly removed by hemodialysis.
Pregnancy & Lactation
Minocycline is contraindicated in pregnancy (Pregnancy Category D) due to potential for permanent tooth discoloration and inhibition of bone growth in the fetus. It is also excreted in breast milk and should not be used during lactation.
