Description
Overview of the medicine
Miroflex 5 mg Tablet contains Baclofen, which is a muscle relaxant used to treat muscle spasms, stiffness, and pain caused by certain conditions like multiple sclerosis, spinal cord injury, or other neurological disorders. It works by decreasing nerve impulses that cause muscle spasms.
Uses & Indications
Dosage
Adults
Initial dosage is 5 mg three times daily, preferably with meals. The dose should be gradually increased at 3-day intervals by 5 mg three times daily, up to a maximum of 20 mg three times daily (60 mg/day). Some patients may require up to 80-100 mg/day.
Elderly
Start with a lower dose (e.g., 5 mg twice daily) and increase gradually under close medical supervision due to increased sensitivity to side effects.
Renal_impairment
Dosage should be reduced in patients with impaired renal function. Consult physician for specific recommendations based on creatinine clearance.
How to Take
Take Miroflex 5 mg Tablet orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water. Do not crush, chew, or break the tablet.
Mechanism of Action
Baclofen is a gamma-aminobutyric acid (GABA) derivative that acts as an agonist at GABA-B receptors. By activating these receptors, it hyperpolarizes afferent terminals and inhibits both monosynaptic and polysynaptic reflexes at the spinal cord level. This leads to a reduction in muscle spasticity and associated pain.
Pharmacokinetics
Onset
Muscle relaxant effects may begin within hours of the first dose, with peak effects observed after several days of treatment.
Excretion
Primarily excreted unchanged by the kidneys (70-80% within 72 hours). A small amount is excreted in the feces.
Half life
3-4 hours
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Approximately 5% of the dose is metabolized in the liver by deamination. The major metabolite is beta-(p-chlorophenyl)-gamma-hydroxybutyric acid.

Contraindications
- Hypersensitivity to Baclofen or any component of the tablet
- Acute porphyria

Storage
Store below 30°C in a cool, dry place, protected from light and moisture. Keep out of reach of children.

Lab Monitoring
- Regular monitoring of renal function (creatinine clearance) is recommended, especially in elderly patients or those with kidney impairment.
- Liver function tests may be considered in patients on long-term therapy.
Doctor Notes
- Initiate Baclofen therapy at a low dose and titrate slowly to avoid adverse effects.
- Emphasize the importance of gradual dose reduction to patients to prevent withdrawal symptoms.
- Monitor for CNS depression, especially when co-administered with other sedatives.
