Overview of the medicine
Montelukast 10 mg tablet is a leukotriene receptor antagonist used for the prevention and long-term treatment of asthma and for the relief of symptoms of seasonal and perennial allergic rhinitis.
Uses & Indications
Dosage
Adults
One 10 mg tablet daily, taken in the evening.
Elderly
No dosage adjustment necessary.

How to Take
Oral administration. Take one tablet daily in the evening, with or without food.
Mechanism of Action
Montelukast selectively blocks the action of leukotrienes (LTD4, LTE4, and LTC4) on the CysLT1 receptor in the airways and on proinflammatory cells. Leukotrienes are potent inflammatory mediators released from various cells, including mast cells and eosinophils, and are responsible for bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment, all of which contribute to asthma and allergic rhinitis symptoms.
Pharmacokinetics
Onset
Clinical improvement seen within one day of administration for asthma, and generally within a few days for allergic rhinitis.
Excretion
Excreted almost exclusively in the feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Plasma half-life is 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration achieved in 2-4 hours for 10 mg tablets.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (CYP3A4, CYP2C9, and CYP2A6) in the liver.

Drug Interactions
Rifampin
May decrease montelukast plasma concentrations.
Phenobarbital
May decrease montelukast plasma concentrations.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Management involves symptomatic and supportive care. Gastric lavage or activated charcoal may be considered in some cases.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Montelukast is excreted in breast milk; caution should be exercised when administered to a nursing mother.
