Overview of the medicine
Monalast 5 mg Tablet contains Montelukast, a leukotriene receptor antagonist, used for the prevention and long-term treatment of asthma and for the relief of symptoms of seasonal and perennial allergic rhinitis.
Uses & Indications
Dosage
Adults
Not typically recommended for adults as primary dose (10 mg usually). If prescribed, typically 5 mg once daily in the evening for specific cases or as directed by physician.
Elderly
No dosage adjustment generally required, but physician discretion is advised.
Renal_impairment
No dosage adjustment required for renal impairment.
How to Take
Take orally once daily in the evening, with or without food. For asthma, take in the evening. For allergic rhinitis, the time of administration may be individualized.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, inhibiting the bronchoconstrictor effect of leukotriene D4. Leukotrienes are potent inflammatory mediators released from mast cells and eosinophils that contribute to the pathophysiology of asthma and allergic rhinitis.
Pharmacokinetics
Onset
Within one day for asthma and allergic rhinitis symptoms.
Excretion
Excreted almost exclusively via the bile in feces (>86%), with minimal renal excretion (<0.2%).
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations typically achieved in 2-4 hours for adults.
Metabolism
Extensively metabolized by cytochrome P450 enzymes, primarily CYP3A4, CYP2C9, and CYP2C8, in the liver.
Side Effects
Contraindications
- •Hypersensitivity to the active substance or to any of the excipients.
- •Acute asthma attacks (not indicated for reversal of bronchospasm in acute asthma attacks).
Drug Interactions
Rifampin
May decrease the plasma concentration of Montelukast, requiring careful monitoring.
Phenobarbital
May decrease the plasma concentration of Montelukast.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In overdose, patients may experience abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use in pregnancy only if clearly needed. Montelukast is excreted in human milk, so caution should be exercised when administered to a nursing mother.
