Description
Overview of the medicine
Montelukast 10 mg is a medication used to prevent and treat chronic asthma, and to relieve symptoms of seasonal and perennial allergic rhinitis. It is not used to treat acute asthma attacks.
Uses & Indications
Dosage
Adults
10 mg orally once daily, in the evening, for asthma or allergic rhinitis. For prevention of exercise-induced bronchoconstriction, take at least 2 hours before exercise, not to exceed once daily.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take Montelon 10 mg Tablet orally once daily, preferably in the evening, with or without food. For asthma and allergic rhinitis, consistently take at the same time each day. For exercise-induced bronchoconstriction, take at least 2 hours before exercise.
Mechanism of Action
Montelukast is a selective and orally active leukotriene receptor antagonist that specifically blocks the CysLT1 receptor. This action inhibits the effects of leukotrienes, which are potent inflammatory mediators released from various cells and involved in the pathophysiology of asthma and allergic rhinitis, leading to reduced bronchoconstriction, inflammation, and mucus secretion.
Pharmacokinetics
Onset
Clinical improvement in asthma symptoms typically occurs within one day of starting treatment. For allergic rhinitis, symptom improvement is also observed within 24 hours.
Excretion
Montelukast and its metabolites are primarily excreted in the feces via bile (approximately 86%), with minimal urinary excretion.
Half life
The plasma elimination half-life is approximately 2.7 to 5.5 hours in healthy young adults.
Absorption
Rapidly and well-absorbed after oral administration. Peak plasma concentrations (Tmax) are reached approximately 2-4 hours after administration.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes, primarily CYP3A4, CYP2C9, and CYP2C8. The metabolites are inactive.
Side Effects
Contraindications
- •Known hypersensitivity to montelukast or any of its excipients.
Drug Interactions
Rifampicin
Co-administration with rifampicin, a potent cytochrome P450 enzyme inducer, may decrease montelukast plasma concentrations. Clinical monitoring is advised.
Gemfibrozil
May increase montelukast exposure; however, no dosage adjustment is considered necessary.
Phenobarbital
Co-administration with phenobarbital may decrease the area under the plasma concentration-time curve (AUC) of montelukast by approximately 40%, potentially reducing its therapeutic effect. Monitor clinical response.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, the most common adverse events observed were abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. There is no specific antidote. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefit justifies the potential risk to the fetus, after consulting a healthcare professional. Montelukast is excreted in breast milk; caution should be exercised when administered to a nursing mother.
