Description
Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the maintenance treatment of asthma and to relieve symptoms of allergic rhinitis.
Uses & Indications
Dosage
Adults
For adults (if prescribed 5mg, e.g., for allergic rhinitis): 5 mg once daily, in the evening. For children 6-14 years: 5 mg once daily, in the evening.
Elderly
No dosage adjustment generally required.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally, once daily in the evening for asthma. For allergic rhinitis, take at any time of day. Can be taken with or without food.
Mechanism of Action
Montelukast selectively blocks the cysteinyl leukotriene 1 (CysLT1) receptor, preventing the actions of leukotrienes (LTC4, LTD4, LTE4). Leukotrienes are potent inflammatory mediators released from various cells (e.g., mast cells, eosinophils, macrophages) and are involved in bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment in asthma and allergic rhinitis.

Onset
Within 1 day for asthma; onset for rhinitis can vary.
Excretion
Primarily excreted in feces via bile.
Half life
2.7 to 5.5 hours.
Absorption
Rapidly absorbed from the GI tract.
Metabolism
Extensively metabolized by CYP3A4, CYP2C9, and CYP2C8 enzymes in the liver.

Contraindications
- •Hypersensitivity to montelukast or any component of the tablet
Drug Interactions
Rifampicin
May decrease montelukast levels.
Phenobarbital
May decrease montelukast levels.
Storage
Store below 30°C (86°F) in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose management is symptomatic and supportive. No specific antidote. Common symptoms reported in overdose include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity.
Pregnancy & Lactation
Pregnancy: Category B. Use only if clearly needed after careful assessment of benefits vs. risks. Lactation: Montelukast is excreted in breast milk. Caution should be exercised when administered to a nursing mother.
