Description
Overview of the medicine
Montica 4 mg Chewable Tablet contains Montelukast, a leukotriene receptor antagonist, used for the prevention and long-term treatment of asthma and for the relief of symptoms of seasonal and perennial allergic rhinitis in children aged 2-5 years.
Uses & Indications
Dosage
Adults
Not applicable for this strength; adults typically use 10 mg.
Elderly
No specific dosage adjustment is generally required for elderly patients.

No dosage adjustment is necessary for patients with renal impairment.
Children_2_5_years
Children 2 to 5 years of age: One 4 mg chewable tablet daily in the evening.
How to Take
Administer orally, once daily in the evening. The chewable tablet should be chewed completely before swallowing. Can be taken with or without food.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, thereby inhibiting the physiological actions of leukotriene D4 (and other leukotrienes like C4 and E4) at this receptor. This blockade leads to reduced bronchoconstriction, decreased mucus secretion, and attenuated inflammatory processes in the airways.
Pharmacokinetics
Onset
Clinical improvement in asthma control can be observed within 1 day of initiating treatment.
Excretion
Excreted almost exclusively via bile in feces, with minimal renal excretion (<0.2%).
Half life
Plasma half-life ranges from 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration is achieved in approximately 2-4 hours. Oral bioavailability is about 64%.
Metabolism
Extensively metabolized in the liver primarily by cytochrome P450 isoenzymes CYP3A4, CYP2C9, and CYP2A6.
Side Effects
Contraindications
- •Known hypersensitivity to montelukast or any of the excipients of the tablet.
Drug Interactions
Phenobarbital
May decrease montelukast plasma concentrations, but generally no dosage adjustment is recommended.
Storage
Store below 30°C, away from light and moisture. Keep out of reach of children.
Overdose
In post-marketing experience and clinical studies, there have been reports of acute overdose with montelukast. Symptoms generally included abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Montelukast should be used during pregnancy only if clearly needed. It is excreted in breast milk; caution should be exercised when administered to a nursing mother.
