Description
Overview of the medicine
Moxifloxacin is a fourth-generation synthetic broad-spectrum fluoroquinolone antibacterial agent. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily by intravenous infusion over 60 minutes for 5-21 days, depending on the infection type.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild, moderate, or severe renal impairment, or for patients on hemodialysis or continuous ambulatory peritoneal dialysis (CAPD).
How to Take
Administer by intravenous infusion over a period of 60 minutes. Do not administer as a rapid or bolus injection. It can be diluted with compatible solutions like 0.9% sodium chloride.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Pharmacokinetics
Onset
Rapid, typically within minutes of intravenous infusion.
Excretion
Excreted both renally (approximately 20% unchanged) and hepatically/biliary (approximately 25% unchanged). Total body clearance is about 12 L/h.
Half life
Approximately 12-15 hours, allowing for once-daily dosing.
Absorption
Rapid and complete absorption after oral administration, however, for IV injection, bioavailability is 100%. Peak plasma concentrations are reached shortly after infusion.
Metabolism
Primarily metabolized in the liver via glucuronide and sulfate conjugation. These metabolites are inactive.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolones.
- Patients with a history of QTc prolongation, uncorrected hypokalemia, or taking Class IA or Class III antiarrhythmics.
- History of tendon disorders related to fluoroquinolone administration.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Corticosteroids
Increased risk of tendinitis and tendon rupture when co-administered with fluoroquinolones.
Class IA and Class III Antiarrhythmics
Increased risk of QTc prolongation, concurrent use is contraindicated.
Antacids, Sucralfate, Iron/Zinc Supplements
Reduced absorption if given orally (not applicable for injection, but relevant for oral forms if therapy switched).

Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
In case of overdose, supportive measures should be taken, including monitoring of ECG for QTc prolongation. There is no specific antidote. Hemodialysis or peritoneal dialysis may remove a small amount of drug.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Moxifloxacin is excreted into human milk; therefore, breastfeeding is not recommended.
