Overview of the medicine
Moxifloxacin is a fluoroquinolone antibiotic used to treat a variety of bacterial infections, including those of the respiratory tract, skin, and abdomen.
Uses & Indications
Dosage
Adults
400 mg once daily for 5-21 days, depending on the infection type and severity. Should be swallowed whole with water.
Elderly
No dose adjustment is typically required based on age alone.
Renal_impairment
No dose adjustment is necessary for patients with mild to moderate renal impairment (CrCl >30 mL/min) or for patients on hemodialysis or continuous ambulatory peritoneal dialysis (CAPD).
How to Take
Take orally, with or without food. Swallow the tablet whole with a glass of water; do not crush, chew, or break.
Mechanism of Action
Moxifloxacin inhibits bacterial topoisomerase II (DNA gyrase) and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Variable, typically within 1-2 hours for significant plasma concentrations to be reached.
Excretion
Approximately 20% of the dose is excreted unchanged in urine and 25% in feces. Both renal and hepatic routes contribute to elimination.
Half life
Approximately 12-15 hours, allowing for once-daily dosing.
Absorption
Rapid and well absorbed from the gastrointestinal tract, with an absolute bioavailability of approximately 90%. Peak plasma concentrations are achieved within 0.5-4 hours.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation, forming inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolones
- History of tendinopathy or tendon rupture associated with quinolone use
- Congenital or documented acquired QT prolongation
- Electrolyte disturbances, particularly uncorrected hypokalemia
- Severe hepatic impairment
Drug Interactions
Warfarin
Potential for increased anticoagulant effect, monitor INR.
Corticosteroids
Increased risk of tendinopathy and tendon rupture.
Antacids, sucralfate, didanosine, mineral supplements
May decrease moxifloxacin absorption. Administer moxifloxacin several hours before or after these agents.
Class IA and Class III antiarrhythmics (e.g., quinidine, procainamide, amiodarone, sotalol)
Increased risk of QT prolongation. Co-administration should be avoided.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children.

In case of an overdose, symptomatic and supportive treatment should be initiated. ECG monitoring is recommended due to the risk of QT prolongation. Hemodialysis or peritoneal dialysis may not be effective in removing moxifloxacin.
Pregnancy & Lactation
Not recommended during pregnancy unless the potential benefit outweighs the potential risk to the fetus. Moxifloxacin is excreted into breast milk; therefore, its use is not recommended during breastfeeding.
