Description
Overview of the medicine
Multiparin is an anticoagulant (blood thinner) used to prevent and treat blood clots (deep vein thrombosis and pulmonary embolism) and in certain cardiac conditions.
Uses & Indications
Dosage
Adults
Dosage varies based on indication (prophylaxis or treatment) and patient weight, typically 20-80 mg subcutaneously once or twice daily.
Elderly
No dose adjustment generally required unless severe renal impairment. Monitor closely.
Renal_impairment
Dose adjustment required for severe renal impairment (CrCl < 30 mL/min). Usually 20 mg once daily for prophylaxis, 1 mg/kg once daily for treatment.
How to Take
Administer by subcutaneous injection, usually into the anterolateral or posterolateral abdominal wall. Do not inject intramuscularly.
Mechanism of Action
Potentiates the inhibitory effect of antithrombin III on Factor Xa and thrombin (Factor IIa), with a preferential effect on Factor Xa.

Anti-Xa activity peaks within 3-5 hours after subcutaneous administration.
Excretion
Primarily renal excretion of active and inactive fragments.

Approximately 4.5 to 7 hours (anti-Xa activity) after subcutaneous administration.

Contraindications
- Hypersensitivity to enoxaparin, heparin, or pork products.
- Active major bleeding or conditions with high risk of uncontrolled bleeding.
- History of heparin-induced thrombocytopenia (HIT) within the last 100 days or presence of circulating antibodies.
- Spinal/epidural anesthesia or lumbar puncture in patients on therapeutic anticoagulation due to risk of spinal hematoma.
