Description
Overview of the medicine
Mycofast is an antifungal medication containing Fluconazole, used to treat various fungal infections, particularly those caused by Candida. It is effective against yeast infections of the vagina, mouth, throat, esophagus, abdomen, lung, blood, and other organs.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg as a single oral dose. For oropharyngeal candidiasis: 50-100 mg once daily for 7-14 days. For systemic candidiasis: 400 mg on day 1, then 200-400 mg once daily.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal function. For elderly patients with renal impairment, dose adjustment based on creatinine clearance is required.
Renal_impairment
Dosage should be reduced based on creatinine clearance. For CrCl < 50 mL/min, initial loading dose should be given, then subsequent doses should be halved.
How to Take
Mycofast capsules can be taken with or without food. Swallow the capsule whole with water.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450 enzyme 14α-demethylase, which is essential for ergosterol synthesis. This inhibition leads to the accumulation of 14α-methyl sterols and a decrease in ergosterol, disrupting fungal cell membrane integrity and function.

Clinical effect typically observed within 1-2 days for most infections.
Excretion
Primarily renal; approximately 80% of the dose is excreted unchanged in urine.
Half life
Approximately 30 hours, allowing for once-daily dosing for many indications.
Absorption
Well absorbed orally, with bioavailability >90%. Peak plasma concentrations achieved within 1-2 hours.
Metabolism
Minimally metabolized; approximately 11% of the drug is metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to Fluconazole or other azole antifungals.
- •Co-administration with terfenadine, cisapride, astemizole, pimozide, or quinidine due to risk of QT prolongation.
Drug Interactions
Statins
Increased risk of myopathy and rhabdomyolysis.
Warfarin
Increased prothrombin time (PT), risk of bleeding.
Phenytoin
Increased plasma concentrations of phenytoin.
Rifampicin
Decreased plasma concentrations of fluconazole.
Oral Hypoglycemics
Increased plasma concentration of sulfonylureas, risk of hypoglycemia.
Hydrochlorothiazide
Increased plasma levels of fluconazole.
Cyclosporine, Tacrolimus
Increased plasma levels of these immunosuppressants.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Overdose may result in exaggerated side effects like hallucinations and paranoid behavior. Treatment is symptomatic and supportive. Hemodialysis may be used to reduce plasma levels.
Pregnancy & Lactation
Pregnancy: Not recommended in pregnancy, especially for high doses or prolonged use, due to potential risk of congenital anomalies. Lactation: Fluconazole is excreted in human milk; use with caution. Consult a doctor.
