Overview of the medicine
Mydipin 5 mg Tablet is a calcium channel blocker used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily. May be increased to a maximum of 10 mg once daily after 7-14 days based on clinical response.
Elderly
A starting dose of 2.5 mg once daily should be considered.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take Mydipin 5 mg Tablet orally, once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Mydipin inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This causes vasodilation, primarily in peripheral arterioles, leading to a reduction in peripheral vascular resistance and blood pressure. In angina, it increases myocardial oxygen delivery by dilating coronary arteries and reduces myocardial oxygen demand by reducing afterload.
Pharmacokinetics
Onset
Initial onset of antihypertensive effect is gradual, typically observed within 6-12 hours.
Excretion
Approximately 60% of the administered dose is excreted in the urine as inactive metabolites, and 20-25% via feces.
Half life
Plasma elimination half-life is 30-50 hours, allowing for once-daily dosing.
Absorption
Well absorbed after oral administration, with peak plasma concentrations occurring 6-12 hours post-dose. Absolute bioavailability is estimated to be 64-80%.
Metabolism
Extensively metabolized by the liver into inactive metabolites via CYP3A4.
Side Effects
Contraindications

Drug Interactions
Ciclosporin
Mydipin may increase ciclosporin plasma concentrations. Monitor ciclosporin levels and adjust dose as needed.
Simvastatin
Coadministration with Mydipin may increase simvastatin exposure, increasing the risk of myopathy/rhabdomyolysis. Limit simvastatin dose to 20 mg/day.
CYP3A4 Inducers (e.g., rifampicin, St. John's wort)
May decrease Mydipin exposure, potentially reducing efficacy. Close monitoring is recommended.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
May increase Mydipin exposure, leading to increased risk of hypotension and edema. Dose adjustment may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly a reflex tachycardia. Management involves supportive measures, including monitoring cardiac and respiratory function, elevation of extremities, and administration of IV fluids. Vasopressors may be used if hypotension remains unresponsive to fluid resuscitation. Intravenous calcium gluconate may be beneficial.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether Mydipin is excreted in human milk; caution should be exercised when administered to a nursing mother.
