Overview of the medicine
Atropine ophthalmic ointment is an anticholinergic agent used to dilate pupils (mydriasis) and paralyze the ciliary muscle (cycloplegia) for ophthalmic examinations and treatment of certain eye conditions.
Uses & Indications
Dosage
Adults
For cycloplegic refraction: Apply a 1/2 inch strip of ointment in the conjunctival sac 3 times daily for 1-3 days prior to examination. For uveitis: 1/2 inch strip 1-3 times daily, or as directed by physician.
Elderly
Similar to adults, but use with caution due to increased risk of systemic absorption and sensitivity.
Renal_impairment
No specific dosage adjustment for ocular use; however, monitor for systemic anticholinergic effects if significant absorption occurs.
How to Take
Wash hands thoroughly before and after use. Gently pull down the lower eyelid to form a pocket. Apply a small amount (about 1/2 inch strip) of ointment into the conjunctival sac. Close the eye gently for 1-2 minutes. Apply pressure to the nasolacrimal duct (inner corner of the eye) to reduce systemic absorption. Do not touch the tip of the tube to the eye or any other surface.
Mechanism of Action
Atropine blocks the action of acetylcholine at muscarinic receptors in the iris and ciliary body, leading to mydriasis and cycloplegia.

Onset
Mydriasis usually within 30-40 minutes, cycloplegia within 1-3 hours.
Excretion
Primarily excreted unchanged in urine.
