Description
Overview of the medicine
Myobac 5 mg tablet contains Baclofen, a muscle relaxant used to treat muscle spasms and stiffness (spasticity) caused by certain conditions like multiple sclerosis, spinal cord injury, or other neurological disorders. It works by reducing the excitability of nerve impulses that cause muscle spasms.
Uses & Indications
Dosage
Adults
Initially 5 mg three times daily. Dose may be gradually increased by 5 mg every 3 days as needed, up to a maximum of 80 mg/day in divided doses. Typical maintenance dose is 30-75 mg/day.
Elderly
Lower initial dose (e.g., 5 mg twice daily) and slower titration recommended due to increased sensitivity and potential for adverse effects.
Renal_impairment
Dose reduction required. Initial dose 5 mg once daily, increasing cautiously. Contraindicated in end-stage renal disease unless benefits outweigh risks.
How to Take
Myobac 5 mg tablet should be taken orally with food or milk to minimize gastrointestinal upset. It should be swallowed whole with water, not crushed or chewed. Do not stop taking this medicine abruptly.
Mechanism of Action
Baclofen acts as a GABA-B receptor agonist, particularly at spinal cord level. It inhibits both monosynaptic and polysynaptic reflexes, reducing spasticity and muscle spasms. It hyperpolarizes afferent terminals, inhibiting the release of excitatory amino acids.
Pharmacokinetics
Onset
Within 1-2 hours of oral administration.
Excretion
Primarily excreted unchanged by the kidneys (70-80%) within 72 hours; a small portion is eliminated in feces.
Half life
Approximately 2.5 to 4 hours.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
About 15% is metabolized in the liver to an inactive metabolite.
Side Effects
Contraindications
- •Hypersensitivity to baclofen or any component of the formulation.
- •Peptic ulcer disease (relative contraindication).
- •Epilepsy not well-controlled by therapy (relative contraindication).
Drug Interactions
Antihypertensives
May enhance hypotensive effects, leading to a drop in blood pressure.
Tricyclic Antidepressants (TCAs)
May increase muscle hypotonia.
CNS Depressants (e.g., alcohol, benzodiazepines)
May enhance CNS depression, causing increased drowsiness and sedation.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, nausea, vomiting, muscle hypotonia, respiratory depression, seizures, and coma. Treatment involves supportive care, gastric lavage if recent ingestion, and respiratory support as needed.
Pregnancy & Lactation
Pregnancy Category C. Baclofen should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; use with caution in nursing mothers.
