Description
Overview of the medicine
Myorel 25 mg Tablet contains Tolperisone Hydrochloride, a centrally acting muscle relaxant used to treat muscular spasms and associated pain. It works by inhibiting the release of certain neurotransmitters and stabilizing neuronal membranes.
Uses & Indications
Dosage
Adults
Initially 50 mg three times daily, gradually increased to 150 mg three times daily, depending on individual need and tolerability. For Myorel 25mg, typically 1-2 tablets three times daily.
Elderly
Dosage adjustment not typically required, but caution is advised. Start with a lower dose if needed.
Renal_impairment
Caution is advised. Start with a lower dose and titrate slowly based on clinical response and tolerability.
How to Take
Oral administration. Tablets should be swallowed whole with a glass of water, preferably after meals to reduce gastrointestinal upset.
Mechanism of Action
Tolperisone acts centrally to relax skeletal muscles. Its exact mechanism is not fully understood but is thought to involve stabilization of neuronal membranes, inhibition of voltage-gated sodium and calcium channels, and antagonism of certain spinal reflex pathways. It reduces muscle hypertonia without significant sedative effects.
Pharmacokinetics
Onset
Within 1-2 hours.
Excretion
Primarily excreted via the kidneys as metabolites.
Half life
Approximately 1.5 - 2.5 hours.
Absorption
Well absorbed from the gastrointestinal tract, but undergoes extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and other isoenzymes.
Side Effects
Contraindications
- •Hypersensitivity to Tolperisone or any component of the tablet.
- •Myasthenia gravis.
- •Children below 18 years (unless specifically indicated and approved by a physician).
Drug Interactions
Warfarin
Tolperisone may increase the effect of warfarin; regular monitoring of INR is recommended.
Methocarbamol
May increase the effects of Tolperisone.
Drugs metabolized by CYP2D6
Tolperisone can increase the concentrations of drugs metabolized by CYP2D6 (e.g., thioridazine, tolterodine, venlafaxine, atomoxetine, desipramine, dextromethorphan, metoprolol, nebivolol, perphenazine). Dose adjustment may be necessary.
Centrally acting muscle relaxants (other)
Caution advised due to potential additive effects.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, gastrointestinal symptoms (nausea, vomiting), and in severe cases, convulsions. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy, especially in the first trimester, unless clearly necessary and benefits outweigh risks. Avoid during breastfeeding as it is unknown if Tolperisone is excreted in breast milk.
