Description
Overview of the medicine
Myoson 50 mg Tablet contains Tolperisone Hydrochloride, a centrally acting muscle relaxant used to treat pathological hypertonus of striated muscle due to neurological diseases (e.g., multiple sclerosis, myelopathy, encephalomyelitis) and in spasticity following stroke, as well as musculoskeletal disorders (e.g., spondylosis, spondylarthrosis, lumbago, cervicobrachial syndrome).
Uses & Indications
Dosage
Adults
Usually 50-150 mg three times daily, depending on individual need and tolerability.
Elderly
Dosage should be individualized, starting with a lower dose and carefully monitoring for adverse effects.
Renal_impairment
Severe renal impairment: Not recommended. Moderate renal impairment: Dosage reduction required, individual titration.
How to Take
To be taken orally, preferably after meals, with a glass of water.
Mechanism of Action
Tolperisone acts on the central nervous system. It primarily reduces muscle tone by inhibiting voltage-gated sodium and calcium channels, which in turn decreases the release of excitatory neurotransmitters from the primary afferent nerve endings. It also has a weak alpha-adrenergic blocking effect. It does not cause sedation.

Excretion
Excreted primarily via the kidneys as metabolites.
Half life
Approximately 1.5-2.5 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentration reached within 0.5-1.5 hours after oral administration. Undergoes extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver and kidneys, primarily by CYP2D6 and other enzymes.
Side Effects
Contraindications
- Hypersensitivity to Tolperisone or any component of the formulation.
- Myasthenia gravis.
- Children and adolescents (due to lack of sufficient data).
Drug Interactions
Methocarbamol
May increase the effect of Tolperisone.
CYP2D6 substrates
Tolperisone is a mild inhibitor of CYP2D6, so caution with drugs metabolized by CYP2D6 (e.g., thioridazine, tolterodine, venlafaxine, atomoxetine, desipramine, dextromethorphan, metoprolol, nebivolol, perphenazine).
Other centrally acting muscle relaxants
Caution advised as their effects may be additive.

Symptoms of overdose may include drowsiness, gastrointestinal symptoms (e.g., nausea, vomiting, epigastric pain), and in severe cases, convulsions. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it is unknown if it is excreted in breast milk.
