Description
Overview of the medicine
Myrica-ER 825 mg Tablet is an extended-release formulation of Naproxen Sodium, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, inflammation, and stiffness caused by various conditions like arthritis, spondylitis, and menstrual pain.
Uses & Indications
Dosage
Adults
Typically, one 825 mg tablet once daily. Dosage may be adjusted based on patient response and tolerability, not exceeding 1650 mg/day.
Elderly
Use with caution; lower doses may be considered due to increased risk of adverse effects, especially GI bleeding.
Renal_impairment
Contraindicated in severe renal impairment. In moderate impairment, reduce dose and monitor renal function closely.
How to Take
Swallow the tablet whole with a full glass of water. Do not crush, chew, or break the tablet. Can be taken with food to minimize stomach upset.
Mechanism of Action
Naproxen Sodium works by inhibiting the production of prostaglandins, chemicals in the body that cause inflammation, pain, and fever. It non-selectively inhibits both COX-1 and COX-2 enzymes.
Pharmacokinetics
Onset
Approximately 1-2 hours for initial effect, sustained for up to 24 hours due to ER.
Excretion
Primarily excreted in the urine (about 95% as metabolites and unchanged drug); a small amount is excreted in feces.
Half life
Approximately 12-17 hours.
Absorption
Well absorbed from the gastrointestinal tract; extended-release formulation provides sustained release.
Metabolism
Extensively metabolized in the liver by demethylation and hydroxylation, followed by conjugation.
Side Effects
Contraindications
- Known hypersensitivity to Naproxen or other NSAIDs
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe heart failure
- Severe renal impairment
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery
Drug Interactions
Lithium
Increased lithium plasma concentrations.
SSRIs/SNRIs
Increased risk of gastrointestinal bleeding.
Methotrexate
Increased methotrexate toxicity.
ACE inhibitors/ARBs
Reduced antihypertensive effect; increased risk of renal impairment.
Diuretics (e.g., Furosemide)
Reduced diuretic and antihypertensive effects.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, heartburn, indigestion, nausea, vomiting, epigastric pain, and rarely gastrointestinal bleeding. Management involves symptomatic and supportive care. Gastric lavage or activated charcoal may be considered within an hour of ingestion. Monitor renal function and acid-base balance.
Pregnancy & Lactation
Pregnancy Category C in early pregnancy, Category D in the third trimester. Avoid use in late pregnancy due to potential for premature closure of the ductus arteriosus. Use with caution during lactation; Naproxen is excreted in breast milk.
