Overview of the medicine
Myrox 6 mg Pediatric Drop contains Montelukast, a leukotriene receptor antagonist used for the prevention and long-term treatment of asthma and for the relief of symptoms of seasonal and perennial allergic rhinitis in pediatric patients.
Uses & Indications
Dosage
Adults
Not applicable for pediatric drops. Adults typically use 10 mg tablets.
Elderly
Not applicable for pediatric drops.
Pediatric
For children 6 months to 5 years of age: 4 mg Montelukast (approximately 0.67 mL of Myrox 6 mg/mL Pediatric Drop) orally once daily in the evening. Administer using the provided calibrated dropper. For children 6 to 14 years, a 5 mg chewable tablet is typically prescribed.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Oral administration. Administer once daily in the evening, with or without food. Use the calibrated dropper provided with the product for accurate dosing. Can be mixed with a spoonful of soft food (e.g., applesauce, mashed carrots) or breast milk/formula at room temperature. Do not mix with other liquids.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, thereby inhibiting the actions of cysteinyl leukotrienes (LTC4, LTD4, LTE4). These leukotrienes are potent inflammatory mediators that contribute to bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment in asthma and allergic rhinitis.
Pharmacokinetics
Onset
Clinical improvement usually seen within one day.
Excretion
Excreted primarily in feces via bile (approximately 86%), with minimal renal excretion (less than 0.2%).
Half life
Approximately 2.7-5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration achieved in 2-4 hours for drops.
Metabolism
Extensively metabolized in the liver by CYP3A4, CYP2C9, and CYP2C8.
Side Effects
Contraindications
- •Hypersensitivity to Montelukast or any component of the formulation.
- •Acute asthma attacks (not indicated for reversal of bronchospasm in acute asthma).
Drug Interactions
Rifampin
May significantly decrease Montelukast plasma concentrations by inducing CYP3A4, CYP2C9, and CYP2C8.
Gemfibrozil
May increase Montelukast plasma concentrations (minor interaction).
Phenobarbital
May decrease Montelukast plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose in adults and children include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive. No specific antidote is known.
Pregnancy & Lactation
Not applicable for pediatric drops. For adult use: Montelukast is pregnancy category B. Use during pregnancy only if clearly needed. Excreted in breast milk; caution advised during lactation.
