Overview of the medicine
Nystatin is a polyene antifungal antibiotic used to treat fungal infections, particularly those caused by Candida species. It works by binding to sterols in the fungal cell membrane, altering its permeability and causing cell leakage.
Uses & Indications
Dosage
Adults
Oral Candidiasis: 500,000 units (1 tablet) 4 times daily, held in the mouth for as long as possible before swallowing. Intestinal Candidiasis: 500,000 to 1,000,000 units 3-4 times daily.
Elderly
No specific dosage adjustment required; use adult dosage.
Renal_impairment
No dosage adjustment needed due to minimal systemic absorption.
How to Take
For oral candidiasis, the tablet should be slowly dissolved in the mouth or held in the mouth for as long as possible before swallowing, to allow prolonged contact with the affected areas. For intestinal candidiasis, the tablet should be swallowed whole.
Mechanism of Action
Nystatin binds to ergosterol, a primary component of the fungal cell membrane, leading to the formation of pores and increased permeability. This results in leakage of intracellular components like potassium and other essential nutrients, ultimately causing fungal cell death.

Excreted almost entirely unchanged in the feces.

Systemic half-life is not clinically relevant due to negligible absorption.
Absorption

Overdose

Pregnancy Category B. Nystatin is generally considered safe during pregnancy as it is not significantly absorbed from the GI tract. Consult a doctor before use. It is unknown whether Nystatin is excreted in human milk, but due to poor oral absorption, it is generally considered compatible with breastfeeding.
