Description
Overview of the medicine
N-Sol 5 (Dextrose 5% w/v) is an intravenous infusion solution containing glucose, primarily used as a source of fluid and calories, to treat hypoglycemia, and as a vehicle for other compatible medications. It helps in maintaining fluid balance and provides energy.
Uses & Indications
Dosage
Adults
Individualized based on fluid, electrolyte, and caloric requirements. Typically, 500-1000 mL administered slowly by intravenous infusion. Max infusion rate should not exceed 0.5 g/kg/hr.
Elderly
Careful monitoring required, similar to adults but considering age-related renal/cardiac function.
Renal_impairment
Use with caution. Fluid and electrolyte balance must be closely monitored to avoid fluid overload and electrolyte disturbances.
How to Take
For intravenous infusion only. Administer slowly into a large peripheral or central vein. Do not administer intramuscularly or subcutaneously. Ensure aseptic technique.
Mechanism of Action
Dextrose is metabolized via glycolysis, which provides energy to the cells. It increases blood glucose levels and supplies free water. The glucose is converted into glucose-6-phosphate, which then enters the metabolic pathways for energy production or storage as glycogen.
Pharmacokinetics
Onset
Rapid, within minutes of infusion.
Excretion
Excreted as carbon dioxide (via lungs) and water (via kidneys).
Half life
Variable, depends on metabolic rate and clinical condition.
Absorption
Administered intravenously, hence 100% bioavailable immediately.
Metabolism
Metabolized to carbon dioxide and water via glycolysis and Krebs cycle.
Side Effects
Contraindications
- •Known allergy to dextrose or corn products
- •Anuria (absence of urine production)
- •Intracranial or intraspinal hemorrhage
- •Severe dehydration without electrolyte replacement
- •Diabetic coma unless insulin is also administered
Drug Interactions
Insulin
Dextrose may counteract the hypoglycemic effect of insulin.
Diuretics
Certain diuretics may alter fluid and electrolyte balance, requiring careful monitoring.
Blood products
Do not administer simultaneously with blood through the same administration set due to risk of pseudoagglutination or hemolysis.
Corticosteroids
May increase blood glucose levels, requiring adjustment of dextrose dosage or insulin.
Storage
Store below 30°C. Protect from light and freezing. Do not use if solution is cloudy, discolored, or contains particulates. For single use only.
Overdose
Symptoms may include fluid overload, electrolyte disturbances (e.g., hypernatremia, hypokalemia), hyperglycemia, osmotic diuresis leading to dehydration, and in severe cases, cerebral edema. Management involves discontinuing the infusion, correcting fluid and electrolyte imbalances, and administering insulin if hyperglycemia is severe.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation when medically indicated. Monitor fluid and electrolyte balance closely. Consult a healthcare professional.
