Overview of the medicine
Nabu 500 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by conditions such as arthritis, menstrual pain, and muscle aches. It works by blocking the body's production of certain natural substances that cause inflammation.
Uses & Indications
Dosage
Adults
500 mg twice daily, or 500 mg initially followed by 250 mg every 6-8 hours as needed, not exceeding 1250 mg on the first day and 1000 mg daily thereafter. For acute gout, 750 mg initially, then 250 mg every 8 hours until the attack subsides.
Elderly
Use the lowest effective dose for the shortest possible duration. Monitor for adverse effects, especially GI bleeding and renal impairment.
Renal_impairment
Not recommended in patients with severe renal impairment (creatinine clearance <30 mL/min). Lower doses may be required for moderate impairment, with careful monitoring.
How to Take
Orally, with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a full glass of water. Do not crush, chew, or break.
Mechanism of Action
Naproxen works by reversibly inhibiting the cyclooxygenase (COX-1 and COX-2) enzymes, which are involved in the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever. By reducing prostaglandin synthesis, naproxen reduces inflammation, pain, and fever.
Pharmacokinetics
Onset
Within 1 hour for analgesia; up to 2 weeks for anti-inflammatory effects.
Excretion
Mainly excreted in the urine (approximately 95%) as naproxen, 6-O-desmethylnaproxen, and their conjugates. A small amount is excreted in feces.
Half life
Approximately 12-17 hours.
Absorption
Rapidly and completely absorbed from the GI tract after oral administration. Peak plasma concentrations are achieved within 2-4 hours.
Metabolism
Primarily metabolized in the liver by the cytochrome P450 system (CYP2C9) to 6-O-desmethylnaproxen, then extensively conjugated.

Contraindications
- Hypersensitivity to naproxen or other NSAIDs (e.g., aspirin-induced asthma, urticaria)
- Active gastrointestinal bleeding or peptic ulcer disease
- Severe heart failure
- Severe renal impairment
- Third trimester of pregnancy
- Coronary Artery Bypass Graft (CABG) surgery

Lithium
Increased lithium plasma concentrations, leading to toxicity.
Methotrexate
Increased methotrexate toxicity.

Overdose
Symptoms may include drowsiness, heartburn, nausea, vomiting, epigastric pain, GI bleeding. Severe overdose may lead to renal dysfunction, hypotension, respiratory depression, and coma. Management is supportive; gastric lavage or activated charcoal may be considered if recent ingestion. Hemodialysis is not effective.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use only if clearly needed during first and second trimesters. Naproxen passes into breast milk; use with caution or consider alternatives during lactation.
