Description
Overview of the medicine
Nabumetone is a non-steroidal anti-inflammatory drug (NSAID) used for the symptomatic treatment of osteoarthritis and rheumatoid arthritis. It provides pain relief and reduces inflammation.
Uses & Indications
Dosage
Adults
Usually 750-1500 mg once daily or in two divided doses. Maximum 2000 mg/day.
Elderly
Lower doses may be considered; monitor renal function.
Renal_impairment
Use with caution; dose adjustment may be necessary in severe impairment. Not recommended for severe renal impairment.
How to Take
Oral. Should be taken with food or milk to reduce gastrointestinal upset.
Mechanism of Action
Nabumetone is a prodrug that is metabolized to 6-methoxy-2-naphthylacetic acid (6-MNA), which is an active NSAID. It inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of pain and inflammation.

Excretion
Primarily excreted in the urine as inactive metabolites and their conjugates, with a small amount in feces.
Half life

Well absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver to the active metabolite 6-MNA and other inactive metabolites.

Contraindications
- Hypersensitivity to nabumetone or other NSAIDs
- Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery
- Active gastrointestinal bleeding or peptic ulcer
- Severe hepatic or renal impairment
- Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Warfarin
Increased risk of bleeding.
Methotrexate
Increased methotrexate toxicity.
Diuretics/ACE inhibitors
Reduced antihypertensive effect; increased risk of renal impairment.

Store below 30°C, in a dry place, away from light. Keep out of reach of children.

Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain. Management is supportive; gastric lavage, activated charcoal may be considered. Monitor renal and hepatic function.

Avoid in the third trimester of pregnancy due to risk of premature closure of the ductus arteriosus. Use with caution in first and second trimesters. Excreted in breast milk, use with caution or avoid during lactation.
