Overview of the medicine
Napasin (Nimesulide) is a non-steroidal anti-inflammatory drug (NSAID) primarily used for its analgesic and antipyretic properties. It's often prescribed for acute pain, osteoarthritis, and primary dysmenorrhea. It exhibits a preferential COX-2 inhibitory action.
Uses & Indications
Dosage
Adults
100 mg twice daily, after food.
Elderly
Generally, the lowest effective dose should be used. Dosage adjustment might be necessary based on renal function.
Renal_impairment
Contraindicated in severe renal impairment. In moderate impairment, dosage reduction might be required.
How to Take
Should be taken orally after food to minimize gastrointestinal discomfort. Do not exceed the recommended dose or duration of treatment.
Mechanism of Action
Nimesulide selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces the production of these inflammatory mediators.

Mainly excreted in urine (around 50-65%) and feces (around 35-50%).

Absorption

Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 2C9 (CYP2C9), into a major active metabolite, 4'-hydroxy-nimesulide.

Drug Interactions

Increased risk of gastrointestinal ulceration or bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves symptomatic and supportive care, gastric lavage, activated charcoal.
Pregnancy & Lactation
Category C in first two trimesters, Category D in third trimester. Contraindicated in the third trimester due to risk of premature closure of the ductus arteriosus and impaired renal function in the fetus. Should be avoided during lactation.
