Description
Overview of the medicine
Naproxen is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and fever. It is commonly prescribed for conditions like arthritis, menstrual cramps, and various types of acute pain.
Uses & Indications
Dosage
Adults
For mild to moderate pain, dysmenorrhea, or acute tendinitis/bursitis: 500 mg initially, followed by 250 mg every 6-8 hours as needed, not exceeding 1250 mg on the first day and 1000 mg/day thereafter. For chronic conditions like arthritis: 250-500 mg twice daily.
Elderly
Use lowest effective dose for shortest duration; monitor renal function due to increased risk of adverse effects.
Renal_impairment
Use with caution; dose reduction may be necessary. Not recommended in severe renal impairment.
How to Take
Take orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a full glass of water. Do not crush, chew, or break.
Mechanism of Action
Naproxen works by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins play a key role in mediating pain, inflammation, and fever.
Pharmacokinetics
Onset
Pain relief typically begins within 1 hour; anti-inflammatory effects may take longer (days to weeks).
Excretion
Mainly excreted in the urine (approximately 95%) as unchanged naproxen, 6-O-desmethylnaproxen, and their conjugates.
Half life
Approximately 12-17 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract; peak plasma concentrations reached within 2-4 hours.
Metabolism
Primarily metabolized in the liver to 6-O-desmethylnaproxen and its conjugates.
Side Effects
Contraindications
- •Known hypersensitivity to naproxen, aspirin, or other NSAIDs
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Active gastrointestinal bleeding or peptic ulcer disease
- •Severe heart failure
- •Severe renal or hepatic impairment
- •Third trimester of pregnancy
Drug Interactions
Lithium
May increase plasma lithium levels, leading to toxicity.
Methotrexate
May increase methotrexate toxicity.
Corticosteroids
Increased risk of GI ulceration and bleeding.
Warfarin and other anticoagulants
Increased risk of bleeding.
Diuretics (e.g., Furosemide, Thiazides)
May reduce natriuretic effect and increase risk of renal impairment.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of gastrointestinal bleeding.
ACE Inhibitors and Angiotensin Receptor Blockers (ARBs)
May reduce antihypertensive effect and increase risk of renal impairment.
Storage
Store at room temperature (below 30°C), protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, heartburn, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Rarely, hypertension, acute renal failure, respiratory depression, and coma may occur. Management involves symptomatic and supportive care, gastric lavage, activated charcoal, and monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category C (D in 3rd trimester or near delivery). Avoid in the third trimester due to potential fetal harm (premature closure of ductus arteriosus). Use during lactation is generally not recommended as naproxen is excreted in breast milk.
