Description
Overview of the medicine
Naprotec 375 mg Tablet contains Naproxen, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, inflammation, and stiffness caused by various conditions such as arthritis, ankylosing spondylitis, tendonitis, bursitis, and menstrual cramps. It works by reducing the production of prostaglandins, which are substances in the body that cause pain and inflammation.
Uses & Indications
Dosage
Adults
For mild to moderate pain, dysmenorrhea, or acute tendonitis/bursitis: 250-500 mg twice daily. For rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis: 250-500 mg twice daily. Maximum daily dose is 1000 mg (or 1500 mg for short-term acute use).
Elderly
Lower doses may be required, and renal function should be monitored. Start with the lowest effective dose.
Renal_impairment
Use with caution. Consider reduced doses and monitor renal function. Avoid in severe renal impairment (CrCl <30 mL/min).
How to Take
Take orally with a full glass of water, with food or milk to minimize gastrointestinal upset. Do not lie down for at least 10 minutes after taking the tablet.
Mechanism of Action
Naproxen inhibits cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. By reducing prostaglandin synthesis, it decreases inflammation, pain, and fever.
Pharmacokinetics
Onset
Analgesic effect typically within 1 hour, anti-inflammatory effects may take up to 2 weeks.
Excretion
Primarily excreted in the urine (about 95% as naproxen, 6-O-desmethylnaproxen, and their glucuronide conjugates). A small amount is excreted in the feces.
Half life
Approximately 12-17 hours, allowing for twice-daily dosing.
Absorption
Rapidly and completely absorbed from the GI tract. Peak plasma concentrations are achieved in 2-4 hours. Food can delay but does not significantly reduce absorption.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes, primarily to 6-O-desmethylnaproxen, which is then glucuronidated.
Side Effects
Contraindications
- •Known hypersensitivity to naproxen, aspirin, or other NSAIDs
- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- •Active gastrointestinal bleeding or peptic ulcer disease
- •Severe renal or hepatic impairment
- •Severe heart failure
- •Third trimester of pregnancy
- •Coronary artery bypass graft (CABG) surgery (peri-operative pain)
Drug Interactions
SSRIs
Increased risk of GI bleeding.
Lithium
Increased plasma lithium levels and toxicity.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of GI ulceration and bleeding.
Other NSAIDs or Aspirin
Increased risk of gastrointestinal adverse effects.
Diuretics (e.g., Furosemide)
Reduced diuretic and antihypertensive effects.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
ACE inhibitors and Angiotensin II receptor blockers
May reduce antihypertensive effect and increase risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, heartburn, indigestion, nausea, vomiting, epigastric pain, and occasionally convulsions. Management involves symptomatic and supportive care. Gastric emptying (e.g., emesis or gastric lavage) may be considered within 4 hours of ingestion. Activated charcoal may be administered.
Pregnancy & Lactation
Pregnancy Category C in first and second trimesters, Category D in third trimester (due to risk of premature closure of fetal ductus arteriosus). Avoid use in late pregnancy. Use during lactation is generally not recommended as naproxen is excreted in breast milk and may cause adverse effects in infants.
