Overview of the medicine
Neforex 20 mg Injection contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is used for the short-term treatment of certain conditions where oral medication is not possible, such as severe reflux esophagitis, Zollinger-Ellison syndrome, and prevention of stress ulcers.
Uses & Indications
Dosage
Adults
Typically 20 mg or 40 mg once daily via intravenous infusion over 20-30 minutes. For Zollinger-Ellison syndrome, initial dose is 60 mg once daily. Doses up to 120 mg/day may be given.
Elderly
No specific dose adjustment is usually required, but caution is advised due to potential age-related decrease in renal or hepatic function.
Renal_impairment
No dose adjustment necessary for patients with impaired renal function.
How to Take
Administered as an intravenous infusion over 20-30 minutes. Not to be given as a rapid bolus injection.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby blocking the final step in gastric acid secretion. This leads to a dose-dependent inhibition of both basal and stimulated acid secretion.

Onset
Within 30 minutes for IV, peak effect within 2 hours.
Excretion
Approximately 80% excreted in urine as metabolites, and the remainder in feces.
Half life
Approximately 0.5-1 hour.
Absorption
Rapidly absorbed from the GI tract after oral administration; bioavailability is about 30-40% due to first-pass metabolism. For IV, bioavailability is 100%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to omeprazole or any components of the formulation, or to other substituted benzimidazoles (e.g., esomeprazole, lansoprazole).
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time may occur. Monitor closely.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel. Concomitant use should be avoided unless absolutely necessary.
Nelfinavir, Atazanavir
Omeprazole decreases the plasma levels of these antiretrovirals, reducing their efficacy. Concomitant use is contraindicated for nelfinavir.
Diazepam, Phenytoin, Citalopram
Increased plasma concentrations of these drugs due to inhibition of CYP2C19 metabolism.
