Description
Overview of the medicine
Fexofenadine is a second-generation antihistamine used to relieve allergy symptoms such as sneezing, runny nose, itchy nose/palate/throat, and itchy/watery eyes, as well as symptoms of chronic idiopathic urticaria like itching and hives.
Uses & Indications
Dosage
Adults
For Seasonal Allergic Rhinitis: 180 mg once daily or 60 mg twice daily. For Chronic Idiopathic Urticaria: 180 mg once daily.
Elderly
No specific dose adjustment generally required, but consider renal function.
Renal_impairment
Initial dose 60 mg once daily for moderate to severe renal impairment. Consult a doctor.
How to Take
Take orally with water, preferably before a meal. Do not take with fruit juice (e.g., grapefruit, orange, apple) as it may reduce absorption.
Mechanism of Action
Fexofenadine selectively inhibits peripheral H1-receptors, preventing histamine from binding and causing allergic reactions. It does not readily cross the blood-brain barrier, resulting in minimal sedative effects.

Excretion
Primarily excreted unchanged in the feces (approx. 80%) and urine (approx. 10%).
Half life
Approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached in 1-3 hours.

Minimally metabolized in the liver (approx. 5% by CYP450 system).

- Hypersensitivity to fexofenadine hydrochloride or any of its excipients.
Drug Interactions
Erythromycin or Ketoconazole
May increase plasma concentrations of fexofenadine. Use with caution.
Antacids containing aluminum or magnesium
May decrease fexofenadine absorption; separate administration by 2 hours.

Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.

Symptoms of overdose may include dizziness, drowsiness, and dry mouth. Management is symptomatic and supportive; hemodialysis does not effectively remove fexofenadine from the blood.
