Overview of the medicine
Neofexim 100 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic. It is effective against a wide range of gram-positive and gram-negative bacteria and is used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Not typically used for adults in suspension form. Adult dosage for Cefixime is generally 400 mg once daily or 200 mg every 12 hours (tablet/capsule).
Elderly
Dosage adjustment is not required in elderly patients unless there is severe renal impairment.
Children
For children 6 months to 12 years: 8 mg/kg/day administered as a single dose or in two divided doses (4 mg/kg every 12 hours). Max 400 mg/day. This 100 mg/5 ml suspension is often given as 10 ml (200 mg) or 5 ml (100 mg) based on weight/age.
Renal_impairment
For creatinine clearance 21-60 mL/min, 75% of the standard dose should be given. For creatinine clearance < 20 mL/min, 50% of the standard dose should be given.
How to Take
Neofexim suspension is for oral use. Shake the bottle well before each use. Use the provided measuring spoon or cup for accurate dosing. It can be taken with or without food.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, leading to bacterial lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50-60%) and bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 30-50% orally absorbed. Food may delay time to peak concentration but does not significantly alter total absorption.
Metabolism
Limited hepatic metabolism; less than 10% is metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- Patients with a history of severe hypersensitivity reactions (e.g., anaphylaxis) to any penicillin.
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
Concomitant administration may increase carbamazepine levels; therapeutic drug monitoring may be necessary.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time (PT) and International Normalized Ratio (INR), potentially leading to bleeding. Close monitoring is recommended.

Store the dry powder below 30°C, away from moisture and light. Store the reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
In case of overdose, gastric lavage may be indicated. Otherwise, treatment is generally supportive and symptomatic. Hemodialysis and peritoneal dialysis do not remove cefixime from the body.
