Neofexim 200 mg Capsule contains Cefixime, a third-generation cephalosporin antibiotic used to treat various bacterial infections including those of the respiratory tract, urinary tract, ear, throat, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg once daily or 100 mg twice daily. For severe infections or uncomplicated gonorrhea, the dose may be increased to 400 mg once daily.
Elderly
No dosage adjustment is generally required in elderly patients unless renal function is significantly impaired.
Renal_impairment
In patients with creatinine clearance less than 20 mL/min, a dose of 200 mg once daily should not be exceeded. In patients with creatinine clearance between 21-60 mL/min, the dose should be reduced to 150-300 mg daily.
How to Take
Neofexim 200 mg Capsule can be taken with or without food. However, taking it with food may help reduce gastrointestinal upset. Swallow the capsule whole with a glass of water. Do not chew or crush it.
Mechanism of Action
Cefixime exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby interfering with the transpeptidation step of peptidoglycan synthesis, which is crucial for maintaining the structural integrity of the bacterial cell wall.

Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Some excretion also occurs via bile/feces.
Half life
Average half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food can delay absorption but does not significantly alter the extent of absorption.
Metabolism
Not significantly metabolized in the liver.

Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity (e.g., anaphylaxis) to any beta-lactam antibiotic (e.g., penicillins).
Drug Interactions
Probenecid
May decrease the renal tubular secretion of cefixime, resulting in increased and prolonged plasma concentrations of cefixime.
Carbamazepine
Concomitant use may increase carbamazepine plasma concentrations. Therapeutic drug monitoring may be necessary.
Oral Anticoagulants (e.g., Warfarin)
Cefixime may increase the prothrombin time and INR, potentially leading to increased bleeding risk. Close monitoring of coagulation parameters is recommended.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in breast milk in small amounts; use with caution in breastfeeding mothers.
