Neopem is a broad-spectrum carbapenem antibiotic used to treat various bacterial infections. It is effective against a wide range of gram-positive and gram-negative bacteria.
Uses & Indications
Dosage
Adults
150 mg orally every 12 hours for 7-14 days, depending on infection severity and type.
Elderly
No specific dose adjustment for elderly patients with normal renal function. Monitor renal function.
Renal_impairment
Dose adjustment required. For CrCl 30-49 mL/min: 75 mg every 12 hours. For CrCl <30 mL/min: 75 mg every 24 hours.
How to Take
Take Neopem-150-mg-tablet orally with a full glass of water, with or without food. It is important to complete the full course of treatment as prescribed by your doctor.
Mechanism of Action
Neopenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. It is highly stable to hydrolysis by most beta-lactamases, including extended-spectrum beta-lactamases (ESBLs).

Onset
Rapid, typically within 1 hour.
Excretion
Primarily renal, with approximately 70% excreted unchanged in urine within 12 hours.
Half life
Approximately 1.5 to 2 hours.
Absorption
Well absorbed orally; peak plasma concentrations achieved within 1-2 hours.
Metabolism
Minimally metabolized; primarily hydrolyzed in the kidney by dehydropeptidase-I, leading to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Neopenem or any other carbapenem antibiotics.
- Severe hypersensitivity reactions to any other beta-lactam antibiotics (e.g., penicillins, cephalosporins).
- Known history of severe renal impairment without dose adjustment.
Drug Interactions
Probenecid
Increases plasma concentrations and half-life of Neopenem, prolonging its effect. Concomitant use should be monitored.
Valproic acid
May decrease valproic acid plasma concentrations, increasing the risk of seizures. Co-administration is generally not recommended.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Keep out of reach of children.

In case of overdose, supportive care and symptomatic treatment should be initiated. Hemodialysis may be useful for removing Neopenem from the system in severe cases.
