Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg every 8 hours, depending on the severity and site of infection, or as directed by a physician.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal function. For renal impairment, dose adjustment is necessary.
Renal_impairment
Dosage adjustment is required based on creatinine clearance. For CrCl 30-50 mL/min: 200 mg every 12 hours; for CrCl 10-29 mL/min: 200 mg every 24 hours.

Mechanism of Action
Neopem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It has a high stability against beta-lactamases produced by many Gram-positive and Gram-negative bacteria.

Contraindications
- Hypersensitivity to Neopem or any other carbapenem antibiotics.

Drug Interactions
Probenecid
Probenecid competes with Neopem for active tubular secretion, resulting in increased plasma concentrations and prolonged half-life of Neopem.
Valproic Acid
Co-administration with Neopem may decrease valproic acid plasma concentrations, potentially leading to subtherapeutic levels. Concurrent use should be avoided or monitored closely.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. Neopem can be removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Neopem is excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.
