Description
Overview of the medicine
Neovitan-100 mg Injection is a parenteral preparation containing a comprehensive blend of B-complex vitamins, crucial for various metabolic processes and nerve function. It is primarily used to prevent and treat deficiencies of these vitamins.
Uses & Indications
Dosage
Adults
Typically 1-2 ampoules (2-4 ml) intramuscularly or intravenously daily or as directed by physician.
Elderly
Same as adult dosage, no specific dose adjustment required unless there are specific renal or hepatic impairments.
Renal_impairment
No specific dose adjustment for mild to moderate renal impairment; use with caution in severe impairment as some metabolites may accumulate. Consult physician.
How to Take
Administered intramuscularly (IM) or slowly intravenously (IV). For IV administration, it may be diluted in normal saline or 5% dextrose solution.
Mechanism of Action
B-complex vitamins act as coenzymes in numerous metabolic pathways, including carbohydrate, protein, and fat metabolism. Thiamine is crucial for energy production, Pyridoxine for amino acid metabolism and neurotransmitter synthesis, Cyanocobalamin for DNA synthesis and nerve myelin formation, and Nicotinamide and Riboflavin for cellular respiration.
Pharmacokinetics
Onset
Relatively rapid, symptoms of deficiency may improve within hours to days.
Excretion
Primarily via renal excretion. Excess amounts are readily excreted in urine.
Half life
Varies for each vitamin, generally short (hours) as they are water-soluble.
Absorption
Rapidly absorbed from the injection site (intramuscular or intravenous) and distributed to tissues.
Metabolism
Metabolized in the liver (e.g., Thiamine is phosphorylated, Pyridoxine is converted to active forms). Water-soluble vitamins are generally not extensively metabolized before excretion.
Side Effects
Contraindications
- •Hypersensitivity to any component of the injection (e.g., thiamine, lidocaine)
- •Known allergy to cobalt (for cyanocobalamin)
- •Leber's disease or tobacco amblyopia (due to risk of optic nerve damage with cyanocobalamin)
Drug Interactions
Levodopa
Pyridoxine may reduce the effectiveness of levodopa (used in Parkinson's disease).
Chloramphenicol
May interfere with the hematopoietic response to Cyanocobalamin.
Phenobarbital, Phenytoin
Pyridoxine may lower serum concentrations of these anticonvulsants.
Isoniazid, Oral contraceptives, Hydralazine, Cycloserine
These drugs may increase the requirement for Pyridoxine.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze.
Overdose
Due to water-solubility, overdose is rare as excess is typically excreted. However, extremely high doses of Pyridoxine can cause peripheral neuropathy. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation when prescribed by a doctor to treat or prevent deficiencies. However, high doses of certain vitamins should be avoided without medical advice.
