Description
Overview of the medicine
Nepofer (Ferric Carboxymaltose) is an intravenous iron preparation used for the treatment of iron deficiency anemia in adults who are intolerant to oral iron or have had an unsatisfactory response to oral iron, or in adults with non-dialysis dependent chronic kidney disease.
Uses & Indications
Dosage
Adults
Typically, a total dose of 1000 mg administered as a single infusion or two infusions of 750 mg at least 7 days apart. Max single dose 1000 mg.
Elderly
No specific dose adjustments needed for elderly patients.
Renal_impairment
No specific dose adjustments needed for patients with non-dialysis dependent chronic kidney disease.
How to Take
Administered intravenously as a slow infusion (over at least 15 minutes for 1000 mg) by a healthcare professional.
Mechanism of Action
Ferric Carboxymaltose is an iron carbohydrate complex that releases iron into the reticuloendothelial system. The iron is then transported by transferrin to the bone marrow for hemoglobin synthesis.
Pharmacokinetics
Onset
Hemoglobin levels begin to rise within 1-2 weeks.
Excretion
Mainly through cellular uptake and utilization; minimal renal excretion.
Half life
Approximately 7-12 hours
Absorption
Rapidly absorbed into the reticuloendothelial system; bioavailable iron reaches the bone marrow.
Metabolism
Iron is removed from the complex and incorporated into the body's iron stores and hemoglobin.
Side Effects
Contraindications
- Hypersensitivity to the active substance or excipients
- Anemia not caused by iron deficiency
- Iron overload or disturbances in iron utilization
- First trimester of pregnancy
Drug Interactions
ACE inhibitors
Potentially increased risk of hypersensitivity reactions with ACE inhibitors, though evidence is limited.
Oral iron preparations
Concomitant administration of oral iron preparations may reduce the absorption of oral iron.
Storage
Store below 30°C. Do not freeze. Keep the vial in the outer carton to protect from light.
Overdose
Overdose may lead to acute iron overload which can manifest as gastrointestinal symptoms, cardiovascular collapse, and liver damage. Treatment involves supportive care and iron chelating agents if severe.
Pregnancy & Lactation
Avoid in the first trimester. Use in second and third trimesters only if clearly needed. Excretion into breast milk is minimal, likely safe during lactation.
