Overview of the medicine
Nerviplex-100-mg-tablet is a combination of B-complex vitamins (B1, B6, B12) indicated for the treatment and prevention of vitamin B deficiencies, especially those associated with neurological disorders like peripheral neuropathy.
Uses & Indications
Dosage
Adults
Usually 1 tablet daily, or as directed by the physician. In severe cases, dosage may be increased.
Elderly
No specific dosage adjustment required, but caution is advised in cases of severe renal impairment.
Renal_impairment
No specific dosage adjustment generally required for mild to moderate impairment. Consult physician for severe renal impairment.
How to Take
Take orally with water, with or without food. It is generally recommended to take with a meal to minimize potential gastrointestinal upset.
Mechanism of Action
These B vitamins act as coenzymes in various metabolic processes essential for carbohydrate metabolism, amino acid metabolism, red blood cell formation, and nerve function. Thiamine (B1) is crucial for energy metabolism, Pyridoxine (B6) for amino acid and neurotransmitter synthesis, and Cyanocobalamin (B12) for DNA synthesis, myelination, and red blood cell maturation.
Pharmacokinetics
Onset
Effects are generally gradual, with clinical improvement observed over days to weeks depending on the severity of the deficiency.
Excretion
Excreted primarily via the kidneys in urine. Excess water-soluble vitamins are generally eliminated rapidly.
Half life
Thiamine: 90-180 minutes (in plasma), Pyridoxine: 15-20 days (for pyridoxal phosphate), Cyanocobalamin: Approximately 6 days (in plasma), with tissue half-life much longer.
Absorption
Readily absorbed from the gastrointestinal tract, primarily in the small intestine. Thiamine is actively transported at low concentrations and passively diffused at high concentrations. Pyridoxine is readily absorbed, and Cyanocobalamin requires intrinsic factor for active absorption from the ileum.
Metabolism
All three vitamins are metabolized primarily in the liver. Thiamine is phosphorylated to active forms. Pyridoxine is converted to pyridoxal phosphate. Cyanocobalamin is converted to methylcobalamin and adenosylcobalamin.
Side Effects
Contraindications
- •Hypersensitivity to any component of the formulation.
- •Levodopa therapy (high doses of pyridoxine can reduce levodopa efficacy).
Drug Interactions
Levodopa
High doses of Pyridoxine (Vitamin B6) may decrease the therapeutic effect of Levodopa, particularly when used without a decarboxylase inhibitor.
Folic Acid/Methotrexate
Large doses of folic acid may mask B12 deficiency.
Phenobarbital/Phenytoin
May alter the metabolism of Pyridoxine.
Isoniazid, Cycloserine, Hydralazine, Penicillamine
These drugs are pyridoxine antagonists and can lead to B6 deficiency.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose is rare due to the water-soluble nature of the vitamins, as excess amounts are readily excreted. However, very high, chronic intake of Pyridoxine (Vitamin B6) can lead to peripheral neuropathy. Symptoms of B6 toxicity include sensory neuropathy, gait disturbance, and numbness.
Pregnancy & Lactation
Generally considered safe during pregnancy and lactation when taken at recommended doses. B vitamins are essential for fetal development and are excreted in breast milk. Consult a physician before use.
