Description
Overview of the medicine
Flunarizine is a selective calcium entry blocker and histamine H1 antagonist, primarily used for the prophylaxis of migraine and symptomatic treatment of vertigo and dizziness.
Uses & Indications
Dosage
Adults
Migraine prophylaxis: Initial dose is 5 mg daily at bedtime. If insufficient, may increase to 10 mg daily at bedtime. Vertigo: 5-10 mg daily at bedtime.
Elderly
Start with 5 mg daily at bedtime due to potential for increased sensitivity and side effects.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe impairment due to metabolic excretion.
How to Take
Take orally, usually once daily at bedtime. It can be taken with or without food.
Mechanism of Action
Flunarizine selectively blocks calcium entry into vascular smooth muscle cells and erythrocyte membranes, thus preventing vasoconstriction and improving microcirculatory blood flow. It also has H1 antagonistic activity.
Pharmacokinetics
Onset
Therapeutic effects for migraine prophylaxis may take several weeks (e.g., 6-8 weeks) to become apparent.
Excretion
Primarily excreted in the feces via biliary excretion (about 60%), with a small amount excreted renally (<1%).
Half life
Long elimination half-life, ranging from 18 to 23 days, allowing for once-daily dosing.
Absorption
Well absorbed from the gastrointestinal tract (bioavailability > 80%). Peak plasma concentrations are reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2D6 and CYP3A4, with significant first-pass effect.
Side Effects
Contraindications
- Hypersensitivity to flunarizine or any component of the tablet.
- Patients with a history of depressive illness or pre-existing Parkinson's disease or other extrapyramidal disorders.
Drug Interactions
Oral Contraceptives
May cause galactorrhea in some women, though the mechanism is not fully understood.
Sedatives, Hypnotics, Alcohol
Concomitant use may lead to increased sedation.
Phenytoin, Carbamazepine, Valproate
These antiepileptics may decrease plasma concentrations of flunarizine.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include sedation, drowsiness, bradycardia, and tachycardia. Treatment is supportive, including gastric lavage and activated charcoal if overdose is recent.
Pregnancy & Lactation
Pregnancy Category C. Should only be used if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as it may be excreted in breast milk.
