Description
Overview of the medicine
Neumig 5 mg Tablet contains Flunarizine, a selective calcium channel blocker, primarily used for the prophylaxis of migraine and symptomatic treatment of vertigo and dizziness of vestibular origin.
Uses & Indications
Dosage
Adults
For migraine prophylaxis: Initially 5 mg once daily at bedtime. If no improvement after 2 months, treatment should be stopped. For vertigo: 5-10 mg once daily at bedtime, depending on response.
Elderly
5 mg once daily at bedtime. If no improvement after 1 month, treatment should be stopped. Special caution is advised due to increased risk of extrapyramidal symptoms.
Renal_impairment
No specific dose adjustment is usually required, but caution is advised in severe renal impairment.
How to Take
Take orally, preferably at bedtime, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Flunarizine selectively blocks calcium entry into cells, reducing excessive influx of calcium. It stabilizes cell membranes, prevents vasoconstriction and hypoxia-induced damage, particularly in cerebral vessels. It also exhibits antihistaminic and antidopaminergic properties.
Pharmacokinetics
Onset
Anti-migraine effect is gradual, usually observed after several weeks of continuous treatment. Vertigo relief can be earlier.
Excretion
Primarily excreted via feces, with minor renal excretion.
Half life
Long, with a terminal elimination half-life of approximately 18-23 days.
Absorption
Well absorbed from the gastrointestinal tract after oral administration.
Metabolism
Extensively metabolized in the liver, primarily via CYP2D6.

Sedatives/Hypnotics/Alcohol
May enhance the sedative effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include sedation, bradycardia, and tachycardia. Treatment is supportive and symptomatic; gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid during lactation as flunarizine may be excreted in breast milk.
