Description
Overview of the medicine
Nexcap 20 mg Capsule contains Esomeprazole, a proton pump inhibitor, used to reduce the amount of acid produced in the stomach. It is effective in treating acid-related conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For Maintenance of Healing of Erosive Esophagitis: 20 mg once daily. For Zollinger-Ellison Syndrome: Initial dose of 40 mg twice daily, adjusted as needed.
Elderly
No dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
Hepatic_impairment
For severe hepatic impairment, a maximum dose of 20 mg once daily is recommended.
How to Take
Take the capsule orally, preferably one hour before a meal. Swallow whole with water; do not chew or crush. If swallowing is difficult, the capsule can be opened and the pellets mixed with a small amount of water or fruit juice (not carbonated) and swallowed immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells, thereby inhibiting the final step in acid production.

Primarily excreted in the urine (about 80%) and feces (about 20%) as metabolites.
Half life
Approximately 1-1.5 hours
Absorption
Rapidly absorbed after oral administration. Bioavailability approx. 64% after a single 40 mg dose, increasing to 89% with repeated once-daily administration. Food delays and decreases absorption.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation
- •Concomitant use with nelfinavir
Drug Interactions
Digoxin
Increased digoxin levels due to increased gastric pH; monitor digoxin levels.
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19. Consider alternative antiplatelet therapy.
Methotrexate
Increased and prolonged serum levels of methotrexate and its metabolite.
Nelfinavir, Atazanavir
Reduced plasma levels of these antiretrovirals, avoid concomitant use.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is limited experience with overdose. Symptoms may include gastrointestinal disturbances, weakness, and central nervous system effects. Treatment is symptomatic and supportive. Esomeprazole is extensively protein bound and is therefore not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
