Nexcap 40 mg Capsule contains Esomeprazole, a proton pump inhibitor used to reduce the amount of acid produced in the stomach. It is prescribed for conditions like acid reflux, heartburn, and stomach ulcers.
Uses & Indications
Dosage
Adults
Gastroesophageal Reflux Disease (GERD): 20 mg or 40 mg once daily for 4 to 8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as per patient response.
Elderly
No specific dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is recommended for patients with renal impairment.
How to Take
Take Nexcap 40 mg Capsule orally, at least one hour before a meal. Swallow the capsule whole with water; do not chew or crush it. If you have difficulty swallowing, you may open the capsule and mix the pellets with a small amount of applesauce and swallow immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cells. This inhibition prevents the final step in gastric acid production, leading to a reduction in both basal and stimulated acid secretion.
Pharmacokinetics
Onset
Within 1 hour.
Excretion
Approximately 80% is excreted in the urine and 20% in the feces as inactive metabolites.
Half life
Plasma elimination half-life is approximately 1.5 hours, but the effect on acid secretion lasts for more than 24 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose, increasing to 89% with repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time have been reported, monitor for bleeding.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19.
Methotrexate
Possible increase in methotrexate levels and toxicity; consider temporary withdrawal of esomeprazole.
Ketoconazole, Itraconazole
Reduced absorption due to esomeprazole-induced increase in gastric pH.
Diazepam, Citalopram, Phenytoin
Increased plasma concentrations of these drugs due to inhibition of CYP2C19.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include gastrointestinal disturbances (nausea, vomiting, diarrhea, abdominal pain), headache, and drowsiness. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing mother.
