Overview of the medicine
Nexcital 5 mg Tablet contains Escitalopram, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, generalized anxiety disorder, panic disorder, social anxiety disorder, and obsessive-compulsive disorder.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily, which may be increased to 10 mg after one week, and then to a maximum of 20 mg daily, depending on patient response and tolerability.
Elderly
A lower starting dose of 5 mg once daily is recommended. Maximum recommended dose is 10 mg daily.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft, which leads to improved mood and reduced anxiety.
Pharmacokinetics
Onset
Therapeutic effects are typically observed within 2-4 weeks, though some patients may experience benefits sooner.
Excretion
Excreted predominantly via the urine (renal pathway), with a smaller portion excreted in the feces.
Half life
The elimination half-life is approximately 27-32 hours.
Absorption
Well absorbed orally, independent of food. Absolute bioavailability is about 80%. Peak plasma concentration (Cmax) is reached in approximately 4-5 hours.
Metabolism
Primarily metabolized in the liver via CYP2C19, CYP3A4, and CYP2D6 enzymes to active and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to escitalopram or any component of the formulation.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- •Concomitant use with pimozide.
Drug Interactions
MAOIs
Increased risk of serotonin syndrome.
Warfarin
May alter anticoagulant effects; monitor INR.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
SSRIs/SNRIs/Triptans/St. John's Wort
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, tremor, agitation, somnolence, nausea, vomiting, tachycardia, seizures, and rarely, serotonin syndrome. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Escitalopram is excreted in human breast milk; therefore, caution is advised when administered to a nursing woman.
