Overview of the medicine
Nexovas (Amlodipine) is a dihydropyridine calcium channel blocker primarily used to treat high blood pressure (hypertension) and certain types of chest pain (angina).
Uses & Indications
Dosage
Adults
Initial dose is 5 mg orally once daily. Maximum dose is 10 mg once daily.
Elderly
Consider 2.5 mg once daily as initial dose. Dose titration should be gradual.
Renal_impairment
No dosage adjustment is generally required for patients with renal impairment.
How to Take
Nexovas tablets should be taken orally once daily, with or without food.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This causes peripheral arterial vasodilation and reduced afterload, leading to a decrease in blood pressure. It also dilates coronary arteries and arterioles, increasing myocardial oxygen delivery.

Onset
Onset of action is gradual, with significant blood pressure reduction observed within 6-9 hours.
Excretion
Excreted primarily via urine (approximately 60% as inactive metabolites, 10% as unchanged drug).
Half life
Elimination half-life is 30-50 hours, allowing once-daily dosing.
Absorption
Well absorbed orally, with a bioavailability of 64-90%. Peak plasma concentrations are achieved 6-12 hours after oral dosing.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications

Drug Interactions
Simvastatin
Amlodipine increases simvastatin exposure; limit simvastatin dose to 20 mg daily.
CYP3A4 inducers (e.g., rifampicin, carbamazepine)
May decrease amlodipine exposure, monitor blood pressure.
Strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, clarithromycin)
May increase amlodipine exposure, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management includes symptomatic supportive care, intravenous fluids, and vasopressors if needed.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Amlodipine is excreted in human milk; lactation is not recommended.
