Description
Overview of the medicine
Nitrofurantoin is an antibiotic used to treat and prevent urinary tract infections (UTIs). It works by stopping the growth of bacteria in the urinary tract.
Uses & Indications
Dosage
Adults
Acute UTI: 50-100 mg four times daily for 7 days. Prophylaxis: 50-100 mg once daily at bedtime.
Elderly
Use with caution due to increased risk of renal impairment and other adverse effects. Avoid if CrCl < 60 mL/min.
Renal_impairment
Contraindicated in patients with creatinine clearance < 60 mL/min due to risk of toxicity and inadequate efficacy.
How to Take
Take orally with food to enhance absorption and minimize gastrointestinal upset. Swallow whole; do not crush or chew.
Mechanism of Action
Nitrofurantoin is reduced by bacterial flavoproteins to reactive intermediates that inactivate or damage bacterial ribosomal proteins, DNA, RNA, and other intracellular components. This results in the inhibition of bacterial cell wall synthesis, DNA/RNA synthesis, and other vital metabolic processes.
Pharmacokinetics
Onset
Rapidly excreted in the urine; antibacterial levels achieved within 30 minutes.
Excretion
Approximately 20-25% of the dose is excreted unchanged in the urine, primarily by glomerular filtration and tubular secretion.
Half life
Approximately 0.3 to 1 hour.
Absorption
Well absorbed from the gastrointestinal tract; absorption is enhanced by food.
Metabolism
Rapidly metabolized in most body tissues, except in the urine where it remains active.
Side Effects
Contraindications
- •Hypersensitivity to nitrofurantoin or any component of the formulation.
- •Anuria, oliguria, or significant impairment of renal function (CrCl < 60 mL/min).
- •History of cholestatic jaundice/hepatic dysfunction associated with prior nitrofurantoin use.
- •Infants younger than one month of age.
- •Pregnant patients at term (38-42 weeks gestation), during labor and delivery, or when the onset of labor is imminent.
Drug Interactions
Probenecid and Sulfinpyrazone
Inhibit renal tubular secretion of nitrofurantoin, leading to increased plasma levels and decreased urinary excretion, which can reduce its effectiveness in the urinary tract and increase systemic toxicity.
Magnesium Trisilicate Antacids
May decrease the absorption of nitrofurantoin, reducing its effectiveness.
Live Bacterial Vaccines (e.g., Typhoid vaccine)
Antibiotics may reduce the therapeutic effect of live bacterial vaccines. Administer live bacterial vaccines at least 24 hours prior to the first dose of nitrofurantoin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal disturbances, peripheral neuropathy, and hemolysis in G6PD-deficient individuals. Treatment is supportive; gastric lavage may be useful. Maintain high fluid intake to promote urinary excretion.
Pregnancy & Lactation
Pregnancy Category B. Avoid use at term (38-42 weeks gestation) due to potential risk of hemolytic anemia in the neonate. Excreted in breast milk; use with caution in nursing mothers, especially if infant is G6PD deficient.
