Overview of the medicine
Nilcid (Esomeprazole) is a proton pump inhibitor that reduces gastric acid secretion. It is used to treat gastroesophageal reflux disease (GERD), stomach ulcers, and conditions involving excessive stomach acid.
Uses & Indications
Dosage
Adults
For GERD/Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily with appropriate antibiotics for 7-14 days. For Zollinger-Ellison syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is required for the elderly, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take orally, usually once daily, at least 1 hour before a meal. Swallow capsules whole; do not chew or crush. If difficulty swallowing, capsules may be opened and granules mixed with a small amount of water or soft food.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme system in the gastric parietal cells. This action reduces both basal and stimulated acid secretion, providing effective relief from acid-related conditions.
Pharmacokinetics
Onset
Onset of acid secretion inhibition occurs within 1 hour.
Excretion
Approximately 80% of metabolites are excreted in the urine and the remainder in the feces.
Half life
Plasma half-life is approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability increases with repeated dosing (approx. 64% after single dose, 89% after repeated doses).
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concurrent use with nelfinavir.
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased INR and bleeding risk.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antiretrovirals.
Ketoconazole/Itraconazole
Reduced absorption due to altered gastric pH.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
