Description
Overview of the medicine
Ninacent 150 mg Capsule contains Pregabalin, an anticonvulsant and neuropathic pain agent. It is used to treat various conditions including neuropathic pain, fibromyalgia, and as an adjunctive therapy for partial-onset seizures.
Uses & Indications
Dosage
Adults
Neuropathic Pain: Initial 75 mg twice daily or 50 mg three times daily, may be increased to 150 mg twice daily (300 mg/day) within a week. Max 300 mg twice daily (600 mg/day). Fibromyalgia: Initial 75 mg twice daily, may increase to 150 mg twice daily. Max 225 mg twice daily (450 mg/day). Partial-Onset Seizures: Initial 75 mg twice daily or 50 mg three times daily, may increase to 150 mg twice daily (300 mg/day). Max 300 mg twice daily (600 mg/day).
Elderly
Dosage adjustments may be necessary due to potential age-related decrease in renal function. Start with a lower dose.
Renal_impairment
Dosage must be adjusted based on creatinine clearance. For CrCl < 60 mL/min, lower doses and/or reduced frequency are required.
How to Take
Ninacent capsules should be taken orally with or without food. They should be swallowed whole with water.
Mechanism of Action
Pregabalin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding modulates the release of several excitatory neurotransmitters, thereby reducing neuronal excitability and pain transmission.
Pharmacokinetics
Onset
Variable; pain relief for neuropathic pain may be observed within a week.
Excretion
Primarily excreted unchanged in the urine (approximately 98% of the dose).
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 1.5 hours. Bioavailability is ≥90% and is not affected by food.
Metabolism
Negligible metabolism in humans; less than 0.1% of the dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to pregabalin or any component of the formulation.
Drug Interactions
ACE Inhibitors
May enhance the effects of pregabalin, leading to increased risk of peripheral edema.
Thiazolidinediones (e.g., pioglitazone, rosiglitazone)
May increase the risk of weight gain and peripheral edema when used with pregabalin.
CNS Depressants (e.g., opioids, benzodiazepines, alcohol)
Concomitant use may potentiate CNS depression, leading to increased drowsiness, dizziness, and respiratory depression.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach and sight of children.
Overdose
Symptoms of overdose may include somnolence, confusion, agitation, and coma. Treatment is supportive, and hemodialysis may be considered in severe cases as pregabalin is dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted into human milk; therefore, caution should be exercised when administered to a nursing mother.
