Description
Overview of the medicine
No-Spa 40 mg injection contains Drotaverine, a smooth muscle relaxant used to relieve spasms of the gastrointestinal, genitourinary, and biliary tracts.
Uses & Indications
Dosage
Adults
40-80 mg (1-2 ampoules) intramuscularly or intravenously, 1-3 times daily. Maximum daily dose is 240 mg.
Elderly
No specific dose adjustment is usually required, but caution is advised in elderly patients, especially those with pre-existing cardiac conditions.
Renal_impairment
No dose adjustment is usually required for mild to moderate renal impairment. Use with caution in severe renal insufficiency.
How to Take
No-Spa 40 mg injection should be administered intramuscularly or slowly intravenously by a healthcare professional. For intravenous administration, it may be diluted with 50-100 mL of 0.9% sodium chloride solution.
Mechanism of Action
Drotaverine inhibits phosphodiesterase (PDE) IV, an enzyme specific to smooth muscle cells, leading to an increase in intracellular cyclic adenosine monophosphate (cAMP) and subsequent relaxation of smooth muscles.
Pharmacokinetics
Onset
Within 2-4 minutes intravenously, 10-15 minutes intramuscularly.
Excretion
Primarily excreted via urine (approx. 50%) and feces (approx. 30%) as metabolites.
Half life
Approximately 9-10 hours.
Absorption
Rapid and complete absorption after intramuscular (IM) administration.
Metabolism
Extensively metabolized in the liver via O-demethylation.
Side Effects
Contraindications
- Hypersensitivity to drotaverine or any excipients
- Severe hepatic, renal, or cardiac insufficiency (severe heart failure)
- Hypotension
- AV block II-III
- Children under 1 year of age
Drug Interactions
Levodopa
May decrease the antiparkinsonian effect of levodopa due to its ability to relax smooth muscle and potentially reduce gastric emptying, affecting levodopa absorption.
Papaverine and other smooth muscle relaxants
Increased antispasmodic effect when co-administered.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze.
Overdose
Overdose may cause cardiac arrhythmias and severe hypotension. Management includes supportive and symptomatic treatment, including monitoring vital signs and managing hypotension.
Pregnancy & Lactation
Use with caution during pregnancy only if clearly needed and the potential benefits outweigh the risks. Avoid during lactation as limited data are available on its excretion into breast milk.
